Lifting mammaire

La mastopexie, communément appelée lifting mammaire, est une intervention chirurgicale qui consiste à remodeler le tissu mammaire pour obtenir une forme plus harmonieuse et remontée.

La grossesse et l’allaitement, le vieillissement ou perte de poids peuvent causer un affaissement de la poitrine avec une descente et agrandissement du mamelon et de la glande mammaire, un relâchement de la peau et une perte de volume dans la partie supérieure du sein.

La mastopexie permet de définir la forme des seins d’une manière plus ferme et plus jeune. Elle permet de repositionner le mamelon au centre du sein, de diminuer le diamètre de l’aréole et de redonner une rondeur et une fermeté au sein en enlevant le surplus cutané.

Cette chirurgie peut être combinée à une augmentation du volume du sein par implants ou transfert de graisse, ou bien à une réduction du volume.

Qui est une bonne candidate pour un lifting mammaire ?

Vous êtes peut-être une bonne candidate si vous constatez que :

– Vos seins sont tombants ou semblent « vides », et cela affecte votre confiance en vous

– Vos mamelons se situent au-dessous du pli des seins et/ou pointent vers le bas

– La peau de votre sein est étirée et les aréoles élargies ou asymétriques

– Vos seins sont asymétriques ou ont une déformation

Pour envisager d’avoir une mastopexie dans les meilleures conditions possibles, il est important de présenter un poids stable. En fait, les variations de poids peuvent modifier le résultat de la chirurgie, ainsi que l’excès de poids augmente le risque de complications.

Trois façons différentes de la procédure

Le type d’intervention dépend du volume initial et de la structure cellulaire et glandulaire :
Péri-aréolaire : cicatrice autour de l’aréole, utilisée pour corriger une descente légère du sein
Verticale : cicatrice autour de l’aréole et verticale, utilisée pour corriger une descente modérée du sein.
T à l’envers : cicatrice autour de l’aréole, verticale et sous le sein, utilisée pour corriger une descente importante du sein.

Anesthésie

Une mastopexie nécessite souvent une anesthésie générale. Une consultation préopératoire avec un anesthésiste sera donc organisée avant la procédure.

Une mastopexie péri-aréolaire de manière peu invasive peut être exécuté sous anesthésie locale.

Hospitalisation

Une mastopexie ne nécessite généralement pas d’hospitalisation, une journée de soins ambulatoires est suffisante. Dans de rares cas, une hospitalisation est requise pour un ou deux jours.

Avant la procédure

Après la 1ère consultation, vous aurez le temps d’y réfléchir. Ensuite, un 2ème rendez-vous sera organisé afin de répondre à vos questions supplémentaires, possiblement choisir une combinaison avec une réduction ou augmentation mammaire et fixer une date pour la chirurgie.

Une mammographie et/ou échographie mammaire datant depuis moins d’un an est nécessaire avant toute chirurgie.

Période postopératoire et convalescence

En quittant la clinique, vous auriez des pansements simples qui seront changés la semaine suivante pour la première fois lors de notre consultation. Vous êtes autorisé à prendre une douche environ 7 jours après la chirurgie. Vous devez porter un soutien-gorge de sport (sans fil, avec une large bande élastique, si possible, avec ouverture avant pour plus de confort) jour et nuit pendant 6 semaines.

Il n’est pas recommandé de conduire et de porter du poids (plus de 2-3 kilos) en quittant la clinique. La reprise de l’activité peut être envisagée après 2 semaines selon le type d’activité. Il faut attendre 6 semaines avant de faire du sport.

Breast enlargement, also known as breast augmentation or augmentation mammaplasty is one of the most common procedures in aesthetic plastic surgery. It can be done by placing modern silicone implants or transferring your own fat, also called lipofilling.

There are different reasons why we are asked for a breast enlargement. As an important  female sex characteristic, the breast has a great significance for a woman’s psychological well-being and self-esteem. The subjective and objective perception of the breast is strongly dependent on the ratio of its shape and size to the perceived body portions. The desire for a breast enlargement aims to restore or build up a harmonic female body silhouette.

An aesthetic breast enlargement could be the right treatment, If you want to feel younger or more feminine.

As the decision to enlarge breasts not only is made for aesthetic reasons – nor is a question of age by the way – it can be covered by the health insurance in selected cases.

Who is a breast enlagement for?

A breast enlargement might be right for you if:

  • your breasts are too small, you experienced breast sagging after breastfeeding and pregnancy, you are hung up about the size of your breast because you think it does not match your age or you systematically wear padded bras
  • your breasts are asymmetrical or your breasts have a deformity.

To consider having a breast augmentation in the best possible conditions, it is important to present a stable body weight. In fact, variations in weight can alter the result of the surgery, as well as excess weight increases the risk of complications.

It is important for young women to reach milestones in growth and physical maturity before considering surgery.

Each individual condition will be discussed with Dr. v. Waechter-Gniadek, who can advise you the best.

Two different ways: silicone implants or fat transfer – what kind of breast augmentation is right for me?

A breast enlargement can be done in two principal different ways. The most common method is the insertion of individually selected silicone implants. Alternatively, the breast volume can be reliably increased by transfer of one’s own body fat also called lipofilling.

At our Centre de Chirurgie du Visage, ORL et Plastique in Geneva, we perform both treatment methods.

The choice of the appropriate technique is determined on the one hand by your desire. On the other hand, certain individual requirements must be met for a safe and successful treatment. In some cases, the treatment has to be limited only to a single technique, but sometimes different techniques are possible.

Hyaluronic acid or other filling materials are by the way no longer approved for injection into the breast and are not offered in our clinic for breast enlargement.

A breast enlargement with silicone implants is especially suitable for young and active women. Many of them exercise intensively and have only a low body fat percentage. Here, in most cases, not enough free adipose tissue can be obtained from the existing fat deposits for a reliable resizing of the breast by transfer of own body fat. The insertion of an implant is in this case the right way to fulfill the desire for a larger breast.

This is a question that will be discussed in a personal conversation between you and Dr v. Waechter-Gniadek.

Implant choice and positioning

The type of the implants (round or anatomic / standard weight or Be-lite) and their position (below or above the muscle) will be selected depending on your desire, morphology, skin quality and your sports activity.

Does breast enlargement leave a scar?

In general, an inframammary fold incision is preferred, resulting in hardly visible, short scars. If a lift procedure is associated, the position and the visibility of the scars will vary. In rare cases it is possible to put breast implants by an incision in the lower part of the areola. We do not perform axillary nor umbilical incisions.

We use proven plastic surgery methods to minimize the appearance of scars and, whenever possible we make incisions where they will be easily concealed.

Anaesthesia

Depending on the technique we choose, local or general anaesthesia is needed.

An implant-based breast augmentation requires general anaesthesia. A preoperative consultation with an anaesthesiolgist will therefore be organised before the procedure.

A breast enlargement by fat transfer is usully performed in local anaesthesia.

Hospitalisation

A breast enlargement does not usually require hospitalisation, one-day outpatient care is enough. In rare cases, inpatient treatment is required for one or two days.

Before the procedure

After the 1st consultation you will have time to think it over. Then a 2nd appointment will be arranged in order to answer your additional questions, choose the implants and set a date for the surgery. The choice of implants depends on your wishes, but especially on your body morphology. Concerning the breast implants size, the final decision rests with the surgeon.

An X-ray control (mammography) and/or breast ultrasound dating since less than a year is required before any surgery.

Postoperative period & convalescence

When leaving the clinic, you will have simple dressings that will be changed the following week for the first time at our consultation. You are allowed to shower around 7 days after surgery. You should wear a sports bra (without underwire, with a wide elastic band, if possible, with front opening for greater comfort) day and night for 6 weeks.

It is not recommended to drive and carry weight (more than 2-3 kilos) when leaving the clinic. Activity resumption may be considered after 2 weeks depending on the type of activity. It is necessary to wait 6 weeks before doing sports.

Breast Enlargement by Silicone Implants

Implantation of silicone implants is the most obvious option for breast enlargement. However, there are different ways to put an implant into a breast, just like there are many different brands and types of breast implants to choose from.

During the consultation, we will discuss with you all aspects of the breast enlargement procedure. We have various tools to assist you: You can put on a number of imptants underneath your clothes to see for yourself what the extra volume in your chest will look like. We can also create a digital 3D-simulation of the breast enlargement, based on a photoscan of your body.

With the anatomical shape of modern implants, it is possible to achieve a very natural looking result. Breast enlargement is a highly predictable and safe procedure when performed by experienced surgeon.

Breast Enlargement by autologous fat trasfer

Another way to increase the size of your breasts, which has been in use for several years now, is to use your own body fat, which will be injected into the breasts. This precudere is also known as lipofilling, liposculpture.

During this procedure, the required amount of fat tissue is removed from elsewhere in your body through liposuction. This fat graft is then injected into your breasts via a few mini-incisions of 2 millimetres.

The advantages of this method are:

  • no foreign materials in the body
  • two procedures for the price of one (breast enlargement and liposuction)
  • the result will last a lifetimey
  • your breasts will look and feel perfectly natural.

The major downside of this method is that roughly half of the injected fat will disappear during the recovery period. Circa 4 months after the procedure, this process will stabilise and the results will no longer change. If you decide to want more volume at that point, it is possible to undergo a second lipofilling procedure.
